Penuma, also sold under the brand name Himplant, describes a soft, crescent-shaped silicone implant. Surgeons place it beneath the skin of the penis to increase girth and flaccid length. A US manufacturer developed it, and it holds FDA 510(k) clearance. This makes it the first implant of its kind cleared in the United States for cosmetic enhancement.

Unlike traditional penile prostheses used for erectile dysfunction, Penuma serves a purely cosmetic purpose. It does not treat any underlying medical condition, and men with normal erectile function are the typical candidates. This distinction matters, since the risk-benefit calculation for a purely cosmetic device differs meaningfully from one addressing a genuine functional problem requiring treatment.
How the Procedure Works
Surgeons place the implant during an outpatient procedure. It typically takes 45 to 60 minutes under general anesthesia. First, they custom-fit the crescent-shaped device to the patient's anatomy. Then, they position it beneath the penile skin.
Recovery generally involves some swelling, bruising, and discomfort in the days following surgery, similar to other penile implant procedures. Careful adherence to post-operative instructions, therefore, supports proper healing.
Most surgeons place the incision at the base of the penis, positioned to minimize visible scarring once healed. The implant itself sits beneath the skin but outside the erectile chambers, which means it does not interfere with natural erectile function or sensation in most cases.
Because placement happens outside the erectile tissue itself, patients generally retain full sexual function afterward, distinguishing this procedure clearly from implants that replace or sit within the erectile chambers, which carry a different and generally more significant risk profile overall.
Typical Penuma Implant Results Reported
Manufacturer-reported outcomes describe average girth gains of roughly 1.5 to 2 inches. In addition, patients see a modest flaccid length increase of about 0.5 to 1 inch. Results depend on implant size and individual anatomy. As with any implant, therefore, individual results vary, and not every patient achieves the described average.
Worth noting, these figures come primarily from manufacturer-reported data rather than large, independent, peer-reviewed studies. This does not necessarily mean the numbers lack accuracy, but it does mean patients should treat them as a general guide rather than a guaranteed outcome specific to their own anatomy.

Why SafeFill Clinic Does Not Offer the Penuma Implant
Penuma's FDA clearance applies only within the United States. It carries no CE certification. European countries, including Turkey, require this certification before doctors can legally use a device. For this reason, therefore, we cannot offer Penuma at our Istanbul clinic, regardless of its status elsewhere.
This is not simply a bureaucratic technicality. CE certification requires its own evaluation process, examining safety data specific to the European regulatory framework. A device lacking this certification has not necessarily failed that evaluation, but it also has not passed it, which leaves an open question our clinic prefers not to leave unanswered for our patients.
Beyond the regulatory issue, published complication data also favors an alternative. Reported rates of implant protrusion run considerably higher for Penuma. This holds true compared with CE-certified alternatives that share a similar design philosophy, a difference worth knowing regardless of where you have surgery.
Protrusion, in this context, means the implant edge becomes palpable or visible through the skin, sometimes requiring a revision procedure to correct. Design details such as material softness, edge shape, and how the device integrates with surrounding tissue all influence this specific risk, which is why device design matters just as much as surgical technique.
Peniflex®: Our CE-Certified Alternative
