Himplant and Peniflex both add permanent girth with a soft silicone sleeve under the shaft skin. The main difference is where you can have each one.
Himplant, formerly called Penuma, holds US FDA clearance. However, it has no CE marking, so surgeons in Turkey and Europe cannot use it. In the US, the manufacturer publishes prices of $10,500 to $25,000.

Which girth option lasts longest?
Both silicone sleeves are permanent. Fat transfer also lasts once it settles, although some of the fat absorbs in the first months. Filler, by contrast, fades gradually and needs top-ups.
Which option is reversible?
Filler is the most reversible, because a dissolving enzyme can remove it. A silicone sleeve can come out through surgery. Fat, however, is hard to remove once it has taken.
What Is Himplant, and Why Is It US-Only?
Himplant is the newer brand name of the Penuma implant. Many men still search for Penuma, so this guide uses both names.
The device and the evidence
Himplant is a crescent-shaped silicone sleeve that a surgeon places over the shaft, under the skin. A US company developed it under the name Penuma. Later, the company renamed it Himplant.
Notably, it holds FDA 510(k) clearance for cosmetic use in the United States. However, it has no CE marking. As a result, European and Turkish surgeons cannot legally use it.
The sleeve comes in several sizes, often listed as L, XL and XXL. Then the surgeon chooses the size from your measurements. In other words, a larger label does not mean a bigger result by itself.
Manufacturer-reported outcomes describe girth gains of roughly 1.5 to 2 inches. Furthermore, a 2024 US series of 92 patients reported an average flaccid length gain of about 2.5 cm. Results depend on size and anatomy.
As with any implant, published risks include infection, fluid collection, implant movement and visible edges. Notably, that 2024 series found fewer complications with a side incision than with the traditional one above the penis.
Why SafeFill Clinic does not offer Himplant
Turkey follows European device rules, so a device needs CE certification before surgeons can use it. Himplant lacks that marking. Therefore, no clinic in Turkey can legally offer it, whatever an advert says.
Instead, Dr. Tas uses Peniflex when a silicone sleeve suits a patient. He also offers filler and fat transfer, because a sleeve is not always the right first step.
Questions about Himplant
How much girth does Himplant add?
The manufacturer reports average girth gains of roughly 1.5 to 2 inches. However, the real gain depends on the sleeve size and on how much your skin can stretch.
How many inches does the XXL Penuma add?
XXL describes the sleeve size, which the surgeon matches to your length. So XXL does not add a fixed number of inches. Larger anatomy simply needs a longer sleeve.
Can I have Himplant in Turkey?
No. Himplant has no CE marking, so Turkish surgeons cannot use it legally. In Turkey, the silicone sleeve option is Peniflex.
Peniflex: The CE-Certified Silicone Sleeve
Peniflex uses the same idea as Himplant: a silicone sleeve under the shaft skin. However, its design and approval differ.
Design and sizing
Specifically, Peniflex uses a softer silicone with rounded edges and small perforations. In turn, these holes let fluid drain and help tissue grow into the sleeve.
On the day, Dr. Tas measures your flaccid and stretched length, girth and skin quality. Then he chooses the width and length. Bigger is not better, because an oversized sleeve stretches the skin and shows at the edges.

Above all, the change is circumference, both flaccid and erect. At first, swelling settles between weeks three and six. After that, most men see the lasting shape by month three.

Is Peniflex a copy of Himplant?
Both are silicone sleeves, so the idea is similar. However, a different company makes Peniflex, with a softer, perforated design. It also holds CE certification, which Himplant lacks.
Can Peniflex be removed?
Yes. Although it is permanent by design, a surgeon can remove it through the same hidden incision. Removal usually takes less time than the original operation.

To be fair, Himplant has the larger published record, which counts in its favour. On the other hand, Peniflex is the only sleeve you can legally have in Turkey or Europe.

Fat Transfer and Filler: When a Sleeve Is Not the First Step
Some men want more girth but are unsure about a permanent implant. In that case, two options work without silicone.
HA filler: a reversible preview
Hyaluronic acid filler adds girth through injections, with no incision. As a result, most men return to normal life within days. The gel absorbs gradually, with a half-life of 12 to 24 months.

Fat transfer: your own tissue

A sleeve suits healthy men with normal erections and enough skin. However, some situations call for a different plan or a delay.

Every girth option carries risks. However, the type of risk differs, and so does the fix.

With a sleeve, infection is the most serious risk, because it usually means removal. Therefore, report fever, spreading redness or sudden swelling the same day. In contrast, filler problems are easier to reverse, since the gel can be dissolved.
Removing or Replacing a Penuma or Peniflex Sleeve
Some men already have a sleeve and dislike its size, edges or position. In that case, removal or replacement is possible.

If a Penuma sleeve causes a problem, he can remove it in Istanbul. Afterwards, the tissue needs time to heal before any new implant. Meanwhile, filler or fat can restore some fullness if you want it.

Can you replace my Penuma with Peniflex?
Often, yes, although rarely in the same operation after an infection. Usually, the old sleeve comes out first. Then a new sleeve can follow once the tissue has healed fully.
